Crescent City Beacon Community (Health IT project)

Program Description

The focus of the Crescent City Beacon Community is to reduce racial health disparities and improve control of diabetes and smoking cessation rates by linking technically isolated health systems and hospitals and to empower patients by increasing their access to patient health records. The initiative seeks to build on existing health IT infrastructure and related systems in the parishes of Orleans and Jefferson with an aggregate population of 790,276. Local partners in the CCBC effort include statewide hospital providers including Ochsner Health System, Tulane Medical Center, Interim Louisiana State University Public Hospital (ILH), 504HealthNet (a shared services organization of neighborhood clinic providers), and metro New Orleans school-based health centers.

Louisiana Community AIDS Partnership

Program Description

The Louisiana Community AIDS Partnership (LCAP) is a Community Partnership of the National AIDS Fund (NAF), the Louisiana Office of Public Health HIV/AIDS Program and national, state, and local community organizations working to promote and improve the health and quality of life of Louisianians infected with or affected by HIV/AIDS. The partnership brings together Louisiana-based foundations, individuals, businesses and government agencies to create a multi-pronged approach to strengthen Louisiana’s response to the HIV/AIDS epidemic. The goal of the LCAP partnership is to provide additional support to HIV/AIDS prevention and direct service programs. The Louisiana Public Health Institute is the administrative home of the LCAP funds that help community organizations enhance their capacity to address HIV/AIDS issues within local communities. Maternal and Child Health

Program Description

LPHI’s Maternal and Child Health (MCH) Initiative seeks to improve the health and wellbeing of Louisiana’s women, infants and children. Through partnership with the Louisiana Office of Public Health’s Maternal and Child Health Program, LPHI targets risk factors associated with maternal, infant and child morbidity and mortality including tobacco, breastfeeding, infant safe sleep, obesity, child injury and substance abuse. To support these efforts, LPHI develops strategic and implementation plans to reach the most vulnerable and affected MCH populations in addition to the promotion and development of policies that improve the health and wellbeing of maternal and child health populations. To ensure success of the MCH Initiative, LPHI has partnered with numerous organizations across Louisiana who are committed to better health for MCH populations including The Louisiana Campaign for Tobacco Free Living, Louisiana Tobacco Control Program, Louisiana Council on Obesity Prevention and Management and the Louisiana Maternal and Child Health Coalition. Program Description

Quit With Us, Louisiana is a partnership of The Louisiana Campaign for Tobacco-Free Living and Louisiana Department of Health and Hospitals Tobacco Control Program. The Quit With Us, Louisiana website is a resource for those who want to quit on their own or help their friend or loved one quit, and for healthcare providers who identify that their patients are tobacco users and have a desire to quit. School Health Connection

Program Description

School Health Connection (SHC) is a regional collaborative effort formed after Hurricane Katrina to support the rebuilding and expansion of School Based Health Centers (SBHCs) in the metro New Orleans area. SHC collaborative partners include local and state government and school systems, universities, medical institutions, foundations, and community partners. In 2006, School Health Connection received limited funding to support the construction of SBHCs and enhance physical and behavioral health services. The Louisiana Public Health Institute (LPHI) administers funds provided by the W.K. Kellogg Foundation on behalf of School Health Connection. The Greater New Orleans Pedestrian & Bicycle Program

Program Description

The Greater New Orleans Pedestrian & Bicycle Program is a joint project between the Regional Planning Commission (RPC) and the Louisiana Department of Transportation and Development (DOTD). Based on a program from Wisconsin, it has been developed with the assistance of current and former Louisiana law enforcement officers. The goal of this program is to reduce the number of pedestrian and bicycle involved crashes in the metropolitan area. For more information, visit http://www.norpc.org.

The Louisiana Campaign for Tobacco-Free Living

Program Description

The Louisiana Campaign for Tobacco-Free Living (TFL) and the Louisiana Department of Health and Hospitals Tobacco Control Program (LTCP) coordinate their efforts in tobacco prevention and control by providing statewide coordination of existing tobacco control initiatives, funding innovative community programs for tobacco control, offering services for people who are ready to quit and developing statewide media campaigns to help reduce the excessive burden of tobacco use on the state’s resources and improve the overall health and quality of life in Louisiana.
